Step 1: Submit Your Intake Form

Complete our secure online intake form and upload your service documents and medical records.

You’ll be asked to:

Provide basic service history and contact information
Upload relevant medical records and supporting documents
Acknowledge our privacy policy and terms of service

Step 2: Physician Review

Once we receive your completed intake form, your case is reviewed by a licensed, board-certified physician.

What we do:

Review all submitted documents
Assess medical evidence and service connection
Prepare a Nexus Letter that is clear, accurate, and medically justified

You’ll receive email/SMS confirmation when your case is being reviewed. We never send any PHI or sensitive data via email.

Step 3: Securely Receive Letter

Once your letter is ready, you’ll be notified via email or text.

You can:

Log in to your private, secure portal
Download your completed Nexus Letter
Request revisions (if needed)
View your case history and status
